Appointment

Initial appointments are booked for a hygiene cleaning and dental examination by the doctor. Hygiene appointments are scheduled for six months from the office visit and all other appointments are scheduled based on the recommendations from the Dentist.

Patient Forms

If you are a new patient to our office, please visit our Patient Forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival.  Thank you and please contact our office if you have any questions at all.

Payment Options

For your convenience, we accept cash, personal checks, money orders, and most major credit cards. Payment is expected at the time services are performed.  Feel free to contact our office If your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.

Dental Insurance

Our dental team is dedicated to providing you with the best possible care. We will also assist you with receiving your maximum allowable insurance benefits. However, in order to do this, it's important to understand our financial policy. Dental insurance is a contract between you and your insurance company with specific benefits, the percentage of coverage, deductibles, and a yearly maximum. Knowing your plan will help you fully utilize your insurance and manage your financial obligations.

Our recommended treatment plan is never based on what your insurance will pay, but rather by what is clinically found. Our fees are based upon our professional time dedicated to providing the highest quality of care and our costs associated with that care. Our dental practice files patient insurance claims as a courtesy to our patients; however, all decisions are ultimately the patient's responsibility.

Your co-payment is to be paid the same day that your dental treatments is rendered. Your co-pay is based upon a percentage of the total amount due.
